Please note: Once purchased, your gift certificate is non-refundable. Certificates expire two years from purchase date. Gift certificates are like money, redeemable by any bearer, so please keep in a safe place. Lost certificates cannot be replaced. Gift certificates are transferable. Certificates must be redeemed in person. Credit toward another procedure will be given for any remaining balance on the face value of the certificate. Not redeemable for cash.
